Co-Med needs to collect personal information from you to administer the MigraineHQ Product Familiarisation Program,
to continue to improve it and to assess it. The information we collect from you includes your full name, profession,
practice/s name/s, address/es, phone number/s and email address/es. We collect this information directly from

If the personal information you provide to Co-Med is incomplete or inaccurate, or you withhold personal information,
Co-Med may not be able to register you or administer the program as required. We will hold your personal information for the duration of your participation in the program, or the applicable period required by law, whichever is longer.
Your personal information will be held in a secure database in Australia. Co-Med may use your personal information to
contact you, manage your participation in the program or to assess the MigraineHQ program. Co-Med may provide your
personal information to third parties that administer the program such as HALO, who dispenses AJOVY® to your
patients, or National Home Nurse who will train your patients on how to inject AJOVY®. We may also disclose your
personal information to our professional advisers (website builders) for administrative purposes, or as required by
If you report an Adverse Event, Co-Med or an authorised third party of Co-Med may contact you for more information.
During the program, if Co-Med becomes aware of any Adverse Events related to Teva's product, it is legally obliged
to report the details to Teva. The only personal information that Co-Med will then pass on is the patient's
initials, date of birth, the medication and dose they are taking, the nature of the event and your contact
